Medical, dental and vision coverage, life and disability insurance, and programs to help save for the future, such as 401(k) and Health Savings Account (HSA).

Benefit Eligibility
You are eligible to enroll in UnityPoint Health benefits if you are in a benefits eligible status. You are eligible for benefits if you are a:

  • FULL-TIME team member who is regularly scheduled to work 64 hours or more per pay period. (0.8 – 1.0 FTE)
  • PART-TIME team member who is regularly scheduled to work 32-63 hours or more per pay period. (0.4 – 0.79 FTE)

Team members who are part of a collective bargaining unit are subject to the terms of their collective bargaining agreement.

Health Insurance

2025:  You have the choice between two different HealthPartners health insurance plans, the NETWORK PLAN or the HEALTH SAVINGS PLAN.

2026:  You have the choice between two different Wellmark health insurance plans, the TRADITIONAL PLAN or the HEALTH SAVINGS PLAN.

Both health insurance plans:

  • • Cover the same basic medical services
  • • Cover the same network of providers, hospitals, and health care specialists who deliver quality care according to network standards and have agreed to lower, preferred rates for covered services.

To choose the right plan for you and your family, consider where you’ll get your care and how out-of-pocket costs fit your budget. UnityPoint Health offers lower costs when you use select in-network providers.

If you enroll in the Health Savings Plan, you will be able to participate in a Health Savings Account (HSA) that you can fund with pre-tax dollars and then use those dollars to pay for current or future medical expenses.

Virtual Care

Virtual Urgent Care is available for individuals 2+ who are located in Iowa, Illinois, Nebraska, and South Dakota. Through a video visit, UnityPoint Health Virtual Urgent Care providers can diagnose, recommend treatment, and prescribe medication for minor illnesses such as cold and flu, rashes, sore throat, headaches, allergies, UTI and more.

2026: Regardless of where you live, you can also access virtual care services through Doctor on Demand at DoctorOnDemand.com/Wellmark or by calling 800-997-6196.

Commonly treated conditions through virtual care include cold/flu, cold sores, fever, cough, vomiting, red eye, headache, rash, sore throat, sinus infections and more.

Dental Insurance

You have the choice between two different Delta Dental of Iowa dental plans, the BASIC PLAN or the PREMIER PLAN. Both plans provide coverage for preventive and basic care services. The Premier Plan also covers orthodontia and major services.

The Enhanced Benefits Program (EBP) is available under both plans. The EBP offers additional oral health services to Eligible Covered Persons with qualifying dental or medical conditions.

Delta Dental of Iowa contracts with dentists and other dental care providers to ensure our team members are covered regardless of where they are located.

Vision Insurance

UnityPoint Health offers vision insurance through Avesis. Under vision insurance, you may purchase your eyeglasses and contacts at the eye care provider of your choice. When you use an Avesis network provider, you receive the highest level of plan benefits and have the lowest out-of-pocket costs.

Health Savings Account (HSA)

Team members that enroll in the Health Savings Plan are eligible to participate in an HSA. The HSA is a savings account that you can fund with pre-tax dollars. The money in the HSA can be used to pay for both current and future qualified medical expenses. Medical expenses are defined under the Internal Revenue Code and cover most medical care, dental services, vision care and prescription drugs.

UnityPoint Health contributes an amount to your HSA each year based on your coverage tier.

Flexible Spending Accounts (FSA)

Flexible Spending Accounts (FSAs) give you the opportunity to lower your taxes by paying for eligible health care and dependent care expenses on a pretax basis. You choose whether to participate in one or both of the accounts each year. The FSAs are funded entirely with your contributions, which are made with pretax dollars from your paycheck

UnityPoint Health offers health care, limited use and dependent care flexible spending accounts.

Life Insurance

Basic Life Insurance

UnityPoint Health provides group term life insurance and Accidental Death and Dismemberment (AD&D) insurance automatically at no cost to benefit eligible team members. Coverage is one time base annual pay, up to $250,000.

Note: Physicians and executives should refer to their Benefit Guide for coverage details. 

Voluntary Life Insurance

Benefit eligible team members may purchase additional voluntary life insurance and Accidental Death and Dismemberment (AD&D) insurance at favorable group rates:

  • • Employee coverage can be elected in increments of $10,000 up to the lesser of eight times annual salary or $500,000.
  • • Spouse or domestic partner coverage can be elected in increments of $10,000 up to $250,000.
  • • Children coverage, up to age 26, can be elected as a flat amount of $5,000 or $10,000 per child.
Disability Insurance

UnityPoint Health provides income protection if you become disabled and cannot work due to a non-work-related illness or accidental injury. Short term and long-term disability coverage is provided at no cost to benefit eligible team members.

You can purchase additional long term disability that will increase their monthly benefit should they become disabled.

Legal Insurance and ID Theft Protection

ARAG provides you and your covered dependents access to a nationwide network of attorneys who work with you to address and resolve life’s legal, financial and identity theft issues.

In the event that your identity is stolen, the ARAG Identity Theft Protection program provides full-service restoration.

Accident Insurance

Accident Insurance pays you benefits for specific injuries and events resulting from a covered accident. The amount paid depends on the type of injury and care received. You can purchase coverage for yourself, spouse/domestic partner, and your dependent children under age 26.

The plan also pays a wellness benefit once per calendar year for each family member enrolled when an eligible health screening test is completed.

Critical Illness Insurance

Critical Illness Insurance pays a lump sum benefit if you are diagnosed with a covered illness or condition. You can use the benefit payment for any purpose you choose. You can purchase coverage for yourself, your spouse/domestic partner, and your dependent children under age 26.

The plan also pays a wellness benefit once per calendar year for each family member enrolled when an eligible health screening test is completed.

Hospital Indemnity Insurance

Hospital Indemnity Insurance pays a daily benefit if you have a covered stay in a hospital, critical care unit or rehabilitation facility. The benefit amount is determined by the type of facility and the number of days you stay. You can purchase coverage for yourself, spouse/ domestic partner, and children under age 26.

Adoption Assistance

UnityPoint Health will grant up to $6,000 in reimbursement, per child, for expenses related to the process of the legal adoption of a child to full-time and part-time benefit eligible team members who have completed at least 90 days of employment.

Pet Insurance

Like health insurance for you and your family, pet insurance is coverage for dogs and cats that can help you be prepared for unexpected vet costs. With MetLife pet insurance, you may be able to cover up to 100% of the veterinary care expenses from any licensed veterinarian, specialist, or emergency clinic across the U.S.

Retirement Savings

UnityPoint Health is proud to invest in your financial health and future by providing a 401(k) Retirement Savings Plan. UnityPoint Health provides a core contribution to you that is equal to 2% of your pay. UnityPoint Health also matches 50% for each dollar you contribute, up to the first 6%.

Certain eligible team members may also contribute to a 457(b) Retirement Savings Plan.
